LTC/Blisterpack Program

Blister Pack Program:

For patients with multiple chronic conditions, it can be a burden to keep track of which medication to take and when to take them. Oftentimes, this can lead to patients mixing up or forgetting to take medications. 

The goal of our blister pack program is to lighten this burden. Our pharmacists work closely with patients, patient advocates, and prescribers to streamline your medication list and package doses based on when they should be taken. Our program will improve adherence, prevent medication errors, and optimize drug therapy.

We offer 2 types of blister packs:

• Single blister pack: separate pack for each individual medication.
  
• Multidose blister pack: all maintenance medications are packed together.
